Who is eligible?
You are eligible if you are a full-time employee working 32
hours or more per week. Your eligible dependents include your spouse
and dependent children to age 19 years, or to age 25 years if they are
full-time students, wholly dependent on you for support, or if they
qualify as an IRS tax qualified dependant.
How do you join?
Complete and return the enrollment forms within 90 days of
full time employment to guarantee coverage. Failure to enroll in this
timeframe will be deemed waiver of participation and future coverage
will not be available.
When is coverage effective?
You are covered after 3 months of employment if you are
actively at work, eligible, and all enrollment forms have been
completed. Coverage begins the day following completion of the
3-month period.
Are pre-existing conditions
covered?
Any illness or injury for which you received any medical care,
treatment or medication within 90 days prior to becoming covered under
this Plan is a pre-existing condition. Benefits will be paid for these
conditions after you are free of treatment for 6 months, or after
you have been covered for 12 months.
What type of Life Insurance
benefits are available?
Fond du Lac Reservation provides you with $25,000 of Group
Term Life Insurance and an equal amount of Accidental Death and
Dismemberment Insurance. Look in the Plan Documents section below to

How can I check on an insurance
claim?
To check on a claim status, you can call or Email the Medical

Flexible Benefits Plan
Through the FDL Flexible Benefit Plan,
the employee has an opportunity to increase take home pay by reducing
their taxes, while covering expenses such as daycare, chiropractic
services, eyewear costs, etc. Under the Flexible Benefits Plan, family
medical and dental premiums are automatically deducted from the
employee's paycheck on a pre-tax basis. This election is subject to
the "use it or lose it" rule. Look in the Plan Documents
