WHI Prescription Plan


How the Prescription Plan works

  • Eligible employees will receive a Pharmacy/Medical card.
  • Employee presents Pharmacy card at the time they pick up their prescription. The Pharmacy will check the system for eligibility.
  • Employee will pay pharmacy the coinsurance/co-pay based on type of prescription purchased.

THERE ARE THREE LEVELS OF RX'S  

  1. GENERIC Drugs  --  $5.00 Co-pay
  2. BRAND NAME FORMULARY Drugs  --  20% Co-insurance or $15.00 minimum 
  3. BRAND NAME NON-FORMULARY Drugs   --  20% Co-insurance or $30.00 minimum

GENERIC drugs ... are the least costly for both the Plan and the employee. 

FORMULARY drugs...are a list of preferred drugs that provide a lower cost to the Plan and employee while maintaining quality of health. 

NON-FORMULARY drugs ... are the most costly of the prescription drugs.

MAXIMUM YEARLY CO-INSURANCE is $2,000.
Upon reaching the yearly $2,000 individual maximum co-insurance, the minimum co-pay still applies.
