Health Care Assistant

Pay Basis: Annual
Position Responsibilities:
- The Health Care Assistant works under the direct supervision of a
Registered Nurse or Public Health Nurse to provide the following home-based,
client-centered services:
- Performs routine delegated medical and/or nursing assigned therapy
procedures, such as monitoring vital signs, blood glucose readings, simple
wound care, etc.
- Assists with body positioning or transfers of clients who are not
ambulatory.
- Feeds clients who, because of their condition, are at risk of choking
- Assists with bowel and bladder control devices and training programs
- Provides skin care, including full or partial bathing and foot soaks
- Provides services to maintain the hygiene of the client’s body and
immediate environment, to satisfy nutritional needs, and assists with the
client’s mobility, including movement, change of location, positioning,
bathing, oral hygiene, dressing, and hair care, toileting, bedding changes,
basic housekeeping and meal preparation
- Remind client to take regularly scheduled medications or perform exercises
- Provides household chores in the presence of technically sophisticated
medical equipment or episodes of acute illness or infectious disease
- Assists community members to access appropriate health and social services
resources
- Reports pertinent client health and social changes to the RN or PHN
- Participates in client care planning and care coordination
- Participates in other health-related activities, as assigned by a RN or
PHN
- May provide client transportation, as assigned
- Participates with the PHN Department’s Continuously Quality Improvement
Committee
- Assists with planning, coordination and implementation of a multitude of
public health and community health related events
- Performs other related duties as assigned
- Displays a responsive and professional manner in solving all requests,
complaints, and problems
- Maintains a clean and organized work area.
- Represents the Fond du Lac Band of Lake Superior Chippewa and the
department in a positive and professional manner in the community
- Recognizes that employees have certain personal responsibilities which
affect the operational efficiency of the Fond du Lac Band of Lake Superior
Chippewa
- Required to maintain proper attendance and reports to work on time in
accordance with Reservation Business Committee policy.
- Recognizes that each employee is a representative of the Fond du Lac Band
of Lake Superior Chippewa and is responsible for demonstrating courtesy,
respect, and sensitivity to the needs of every person including visitors and
co-workers.
- Due to changes and modifications in the job from time to time, employees
are required to be flexible and assume other responsibilities assigned by
management as management sees the necessity. Some assignments may not be
listed on this description.
Physical Requirements:
- Bending, stooping, lifting, standing and walking for long periods are
required.
Position Qualifications:
- High school diploma or G.E.D. is preferred
- State Home Health Aide certification is required.
- Valid license, reliable vehicle, a good driving record and car insurance
is required.
- Experience in the Indian community is required.
- Ability to maintain confidentiality of staff/client information is
required.
- Desire to serve as advocate for community. Knowledge and understanding of
cultural, socio-economic, and emotional factors are required.
- Knowledge of normal nutrition, good health habits, and principles of good
hygiene are required.
- Ability to communicate effectively orally and in writing is required.
Computer skills are preferred.
- Ability to establish professional and harmonious working relationships on
all projects and with all parties involved.
- Ability to provide prompt resolution of problems and conflicts as they may
occur is required.
- Subject to pre-employment annual background checks, and annual reference
checks
- Subject to testing in accordance with the Fond du Lac Employee Drug and
Alcohol Policy
- Some travel may be required.
