General Summary:
A case manager’s function is to arrange for consistent and appropriate activates for adult and children clients that will enhance a Diagnostic Assessment performed by a mental health therapist. Their role is to guide the client in coping with their mental health situation, socialization and self sufficiency.
Position Responsibilities:
- Provide mental health case management services to children and adolescents with severe emotional disturbance (SED) and to their families.
- Provide case management services to adults with serious and persistent mental illness (SPMI).
- Assist SED and SPMI clients in obtaining needed mental health, social, educational, medical, vocational, recreational, volunteer, advocacy, transportation and legal services.
- Complete or assist the clients in obtaining a comprehensive diagnostic assessment.
- Develop an Individual Family Community Support Plan (IFCSP) to address the holistic needs of clients.
- Make referrals to inter agency resources and community resources as necessary.
- Attend client staffing at local schools, county social services, community mental health centers and legal systems as necessary.
- Make collateral contacts with other service providers on a regular basis.
- Monitor clients progress towards IFCSP goals, continue to assess client’s needs and appropriateness for services, and make recommendations for additional services as needed.
- Receive clinical supervision of mental health services activities including meeting with clinical supervisor a minimum of one time per month on an individual basis and in a group clinical supervision setting twice per month.
- Attend all court hearings, providing case updates and making recommendations.
- Provide individual clinical counseling to patient on a case by case basis as approved by the Behavioral Health Coordinator.
- Assist in the coordination of out of home placement activities for children or adolescents when this level of intervention is necessary.
- Maintain current and accurate case records on each client.
- Complete Mental Health Case Management eligibility forms and submit billing forms for services.
- Distribute and present information to the community to prevent and create awareness of mental and emotional disturbance.
- Distribute tools and collect data used for program evaluations such as client satisfaction surveys.
- Provide in-service training for staff as needed.
- Attend trainings that may assist in performance of job duties.
- Attend all scheduled agency meetings.
- Submit RBC reports and DHS reports in an accurate, timely manner.
- Appropriate work attire is required.
- Must attend all mandatory department trainings.
- Ensures confidentiality of financial records and all records for employees, band members and/or clients.
- Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.
- Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers.
- Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community.
- Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.
- Maintains a clean and organized work area.
- Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.

Position Qualifications:
- Class D Drivers License required and must be insurable under Fond du Lac’s insurance requirements.
- Bachelor’s Degree in Social Work or related field is required.
- Master’s of Social Work Degree is preferred.
- Experience working with Indian families is required.
- Knowledge of System’s Theory and familiarity with community resources is required.
- Knowledge of computer applications to include word processing, Access, Excel and Microsoft Outlook is required.
- Ability to communicate effectively orally and in writing is required.
- Ability to work independently and establish priorities is required.
- Attention to detail and accuracy is required.
- Ability to establish professional and harmonious working relationships on all projects and with all parties involved.
