Clinic Dietician, Full Time

Location: MNAW & CAIR
Supervisor: Family Nurse Practitioner Coordinator

General Summary:

The clinic dietitian provides nutrition education, counseling and Medical Nutrition Therapy to patients across the lifespan for multiple medical conditions such as obesity, pre-diabetes, diabetes, cardiovascular disease, hypertension, cancer, food allergies and intolerances, etc. and assists with the diabetes programs.

Position Responsibilities:

  • Performs nutrition screening, assessment, counseling, Medical Nutrition Therapy, and education to patients and families.
  • Provides education in both individual and group settings.
  • Facilitates weight management programs.
  • Provides diabetes self-management training and education to individuals and groups on all aspects of diabetes care, using reliable, science-based and culturally relevant education materials and curricula.
  • Documents an assessment, education provided, goals, evaluation of patient’s understanding and plan in the Electronic Health Record.
  • Communicates with primary health care providers as needed to assure quality client care.
  • Participates in the planning, development, marketing and implementation of nutrition and/or diabetes-related programs for clinic clients or groups, staff, and for community outreach.
  • Promotes healthy lifestyle changes throughout the reservation.
  • Develops and updates culturally appropriate education materials and tools.
  • Assists with maintenance of the American Diabetes Association Recognition process and the Indian Health Service Special Diabetes Program for Indians grant applications and Best Practices as directed.
  • Participates in all mandatory trainings and meetings.
  • Participates in Continuous Quality Improvement activities as directed.
  • Assumes responsibilities for personal, professional development and maintains clinical and educational competency.
  • Appropriate work attire is required.
  • Must attend all mandatory department trainings.
  • Ensures confidentiality of financial records and all records for employees and/or clients.
  • Assist with maintenance of the American Diabetes Association Recognition process, and the Indian Health Service Special Diabetes Program for Indians grant applications and Best Practices, as directed.
  • Displays a responsive and professional manner in promptly responding to all requests, complaints, and problems.
  • Recognizes that each employee is a representative of the Fond du Lac Band of Lake Superior Chippewa and is responsible for demonstrating courtesy, respect, and sensitivity to the needs of all others, including visitors and co-workers.
  • Represents the Fond du Lac Band of Lake Superior Chippewa and the department in a positive and professional manner in the community.
  • Required to maintain proper attendance including reporting to work on time in accordance with applicable policies.
  • Maintains a clean and organized work area.
  • Due to changes and modifications in the job from time to time, employees are required to be flexible and assume other responsibilities assigned by management as deemed necessary.

Physical Requirements:

  • Normal physical requirements.

Position Qualifications:

  • Registered Dietitian, Licensed Dietitian in the state of Minnesota is required.
  • Certified Diabetes Educator (CDE) is required; if not certified at the time of hire employee must obtain certification within 24 months of hire, and must maintain certification for the duration of employment.
  • Knowledge in computer applications to include word processing, Access, Excel and Microsoft Outlook is required.
  • One-year experience in clinical nutrition counseling is preferred.
  • Ability to communicate effectively orally and in writing is required.
  • Ability to work independently and establish priorities is required.
  • Attention to detail and accuracy is required.
  • Ability to establish professional and harmonious working relationships on all projects and with all parties involved.
  • Subject to drug and alcohol testing in accordance with the Fond du Lac Reservation’s Personnel Policies.
  • Subject to pre-employment and annual background checks.
  • Some travel is required.
