Native American Nikes Program

HERE’S HOW TO GET YOUR PAIR!

This shoe program is sponsored by the MNAW Diabetes program and is managed by the Cloquet Community Center Staff.

In order to receive a pair of shoes you must be 13 yrs of age or older and:

FDL Enrolled-Show ID

OR

Native American and be eligible to receive services at the MNAW and live in the service area. You will be asked to get an eligibility paper from the MNAW front desk and bring it to the Cloquet Community Center when you come to get your shoes.

SHOES will cost:

$20.00 per pair if you attend three wellness events, click below for brochure and calendars with applicable events listed. Bring your signed attendance sheet with you to qualify for the $20.00 rate.

OR

$50.00 per pair if you do not attend any wellness events.
