Public Health Nursing provides many services and
activities for Fond du Lac Elders. See below for information about all the
various Elder programs.

This is a monthly calendar highlighting special days and events for Fond du
Lac Elders.

Elder Newsletter
A monthly newsletter and calendar are mailed to elders from the Public
Health Nursing Department. The calendar changes monthly and contains healthy
living tips, recipes, exercise classes and event announcements. Click below

Cancer Prevention
Cancer is a major problem for American Indian communities. This program
provides education on all aspects of cancer, promotes cancer screening, and
links people with cancer and their caregivers to supportive resources.
Cancer Researchers don’t have enough information about what contributes to
cancer in American Indians or what interventions might help to reduce cancer
in the population. This program helps gather more information and form
partnerships to address these areas for the Fond du Lac community.
Diabetes Prevention ("On the Move")
Exercise classes, food preparation classes and client information designed
to promote awareness of diabetes and to prevent diabetes in people at risk
to develop it. Classes are made fun through group participation and
incentives. Fitness classes include yoga, stretching, weight lifting,
walking and more.
Diabetes Program Site
Elder Advocacy
Advocacy Services are provided for the elders living within the CAIR service
delivery area. . Services include assistance with health care access,
finding resources, support and companionship, medication delivery and much
more.
Elder Ceramics
A relaxing group class offered monthly on the reservation to all American
Indian elders in the Sawyer and Cloquet Community Centers.
Elder Exercise
Classes are provided twice weekly at the Tribal Center and once a week
at the Washington Center in Duluth. Elders are encouraged to attend the
classes to feel good and prevent health problems. Activities include
walking, swimming, video exercise and chair exercises if needed.
Elder Homebound
Outings
Health Care Assistants and CHRs provide a monthly outing for elders who have
difficulty getting around without assistance. Activities include going to
movies and dinner, attending special events (like the circus), Vista Fleet
cruises and other fun things to do! A meal is provided.
Elder Homebound
Shopping
Health Care Assistants (HCAs) and CHRs transport homebound elders on monthly
shopping trips to Super One and Wal-Mart. The first Thursday of each month
the homebound elders are transported to Super One and the second Thursday
they are transported to Wal-Mart.
Elder Picnic
An annual picnic held at Molstad’s on Big Lake. BINGO and prizes promote fun
and a good time for all.
Foot Care
This is a monthly clinic to help people who have difficulty reaching and
caring for their feet. This clinic is most appropriate for people with
diabetes, arthritis, obesity and other health concerns.
Health Care Assistants
Health Care Assistants provide personal cares (hair washing, showers,
nail care) monitor vital signs, visiting light housekeeping, meal
preparation and other services for clients with medical or health concerns,
as assigned by the Public Health Nurse.
Home Safety
Assessments
Public Health Nursing staff is available to perform a home safety check for
families with young children or for elders. A limited number of safety
items are available free of charge following the home safety check if
needed. Instruction on how to properly use equipment will be provided,
although the nursing staff cannot actually install the equipment for
families.
In-Home Care
Specialized nursing care is provided in the home to assist with medication
management, wound care, dressing changes, post-operative care and so on. The
Public Health Nurses are Registered Nurses and are able to provide medical
care to the client, as ordered by the client’s physician.
Medication
Delivery
Community Health Representatives (CHRs) deliver medical prescriptions daily
for clients who are sick or have no means of getting their prescriptions.
Requests for medication deliveries must be made by 3:00PM or they will be
put on the schedule for the following day.
Medication Management and Set-up
This service is available to Elders who are on the Public Health Nursing
caseload and must take multiple medications. A Public Health Nurse and LPN
can help you categorize your medications; clarify dosages and number of
times to administer them daily and help set them up for a 1-2 week time
period.
Respite and Caregivers
Services are for families with a homebound or disabled elder. A Health Care
Assistant is able to provide personal cares and socialization while the
care giving family member runs errands, attends appointments, etc.
