Adult Services
Public Health Nursing provides many services for
adults in the Fond du Lac community. See below for information about the PHN
adult programs. Adult
Immunizations
Life threatening diseases
may be prevented for children and adults by being properly immunized. The
Public Health Nursing Department provides information and immunization
clinics for all age groups.
Asthma Education
A Public Health Nurse has received advanced training and has been certified
as an Asthma Educator. She is available to assist families with children who
have asthma and adults newly diagnosed with reactive airway disease.
Education is provided on medications, self-management techniques and
controlling environmental triggers.
Cancer Prevention
Cancer is a major problem for Indian communities, but we don’t have enough
information about what contributes to cancer in American Indians. This
program will help us gather more information about how cancer affects the
Fond du Lac community.
Communicable Disease Follow-up
A Public Health Nurse contacts clients who have been diagnosed with a
communicable disease to assure they have received the proper course of
medication and to prevent further health problems.
Community Health Representatives
Community Health Representatives (CHRs) provide transportation for
individuals only after all resources are exhausted. CHRs will transport to
medical, dental, WIC, and optical appointments. Appointments should be
confirmed with the CHRs schedule to assure that clients arrive on time to
their appointment. CHRs will wait for clients and transport them back
home. No one under the age of 18 will be transported without a parent. CHRs
also deliver medications to homebound individuals with no other means of
getting their medications.
Diabetes Prevention ("On the Move")
Exercise classes, food preparation classes and client information designed
to promote awareness of diabetes and to prevent diabetes in people at risk
to develop it. Classes are made fun through group participation and
incentives. Fitness classes include yoga, stretching, weight lifting,
walking and more.
Diabetes Program Site
Foot Care
This is a monthly clinic to help people who have difficulty reaching and
caring for their feet. This clinic is most appropriate for people with
diabetes, arthritis, obesity and other health concerns.
Health Care
Assistants
Health Care Assistants provide personal cares (hair washing, showers, nail
care) monitor vital signs, visiting light housekeeping, meal preparation and
other services for clients with medical or health concerns, as assigned by
the Public Health Nurse.
Home Safety
Assessments
Public Health Nursing staff is available to perform a home safety check for
families with young children or for elders. A limited number of safety
items are available free of charge following the home safety check if
needed. Instruction on how to properly use equipment will be provided,
although the nursing staff cannot actually install the equipment for
families.
In-Home Care
Specialized nursing care is provided in the home to assist with medication
management, wound care, dressing changes, post-operative care and so on. The
Public Health Nurses are Registered Nurses and are able to provide medical
care to the client, as ordered by the client’s physician.
Tobacco Cessation
Tobacco
Cessation is designed to help you quit smoking through four
one-hour individual sessions. You will learn about the challenges of
quitting tobacco and ways to overcome them, so you can quit for good. The
Tobacco Cessation Educator will use a carbon monoxide Smokerlyzer to
help you see the immediate effects smoking has on your body, as well as the
immediate benefits of quitting. Tobacco cessation aides such as nicotine
patches, wellbutrin, Chantix, acupuncture, herbal remedy and self-hypnosis are available for those who
are eligible. If you are interested in learning if you are eligible for
these services, the Tobacco Cessation Educator can refer you to the
appropriate service provider.
