Steps to Success

Patients who register for the program will be scheduled for a 2-hour appointment.  At this appointment they will have a fitness, nutrition, pharmacy and cardiac risk assessment.  Patients must come to this appointment fasting because labs will be drawn.  A light snack will be provided.
At the next appointment patients will meet with their medical provider.  Providers will review the information collected at the initial appointment and set goals with patients.  Patients may be referred to specialty areas like the smoking cessation program or physical therapy depending on what they are ready to work on.
Patients will have a 3-month follow-up appointment with Tiffany Elton. Vitals and labs will be taken and cardiac risk will be recalculated. The goal is to see a reduction in cardiac risk at these appointments. Cardiac risk can be reduced in several ways like quitting smoking, lowering cholesterol, improving blood pressure and improving diabetes control.
