Heart disease is the leading cause of death in Native Americans.

Mino Aya Ode is the Cardiovascular program at Fond du Lac Human Services designed to help you prevent or control heart disease. What you’ll get is support and education to help you make positive changes that will last a lifetime.
Risk factors are traits and lifestyle habits that increase the risk of heart disease. Mino Aya Ode will help you take control of these risk factors and adopt a healthy lifestyle.

Risk factors you CAN'T control:

  • Increasing Age
  • Gender
  • Race
  • Family History
  • Previous Heart Attack or Stroke

Risk factors you CAN control:

  • Smoking
  • High Blood Pressure
  • High Cholesterol
  • Physical Inactivity
  • Weight
  • Diabetes
  • Stress
Work with your health care provider to evaluate and control your risk factors

We understand that changing your lifestyle to reduce these risk factors will not be easy. Change is difficult and takes time. Our team of health professionals would like to help you make these changes.
Over 65 people have joined the Mino Aya Ode program and came for their initial visit. Most have also set goals with their medical provider and have been referred to physical therapy, a personal trainer, cardiovascular clinic, or the dietitian to work towards meeting their goals.

Anyone who is 18 years or older wanting to reduce cardiovascular risk that is eligible to use the Fond du Lac Health Service can join.

Indian clients are not billed for co-pays or deductibles.  Non-Indian clients with FDL Insurance may use the Fond du Lac Health Service but will be billed for co-pays, deductibles and services not covered by their insurance plan.
To register, simply call the medical desk at 879-1227 and join today.  We recommend you schedule your initial appointment and your provider appointment when you register to avoid a long delay between the two visits.
