Q: What is the Mino Aya Ode “Healthy Heart” Program?

A: Mino Aya Ode is a Cardiovascular Program at Fond du Lac Human Services designed to help you prevent or control heart disease.

Q: How does the program work?

A: Patients who register for the program will be scheduled for a 2-hour appointment. At this appointment they will have a fitness, nutrition, pharmacy and cardiac risk assessment. Patients must come to this appointment fasting because labs will be drawn. A light snack will be provided.
    At the next appointment patients will meet with their medical provider. Providers will review the information collected at the initial appointment and set goals with patients. Patients may be referred to specialty areas like the smoking cessation program or physical therapy depending on what they are ready to work on.
    Patients will have a 3-month follow-up appointment with Tiffany Elton. Vitals and labs will be taken and cardiac risk will be recalculated. The goal is to see a reduction in cardiac risk at these appointments. Cardiac risk can be reduced in several ways like quitting smoking, lowering cholesterol, improving blood pressure and improving diabetes control.

Q: Who will help me?

A: We understand that changing your lifestyle to reduce your risk of heart disease will not be easy. Change is difficult and takes time. Our team of health professionals would like to help you make these changes.

Q: Will I lose weight?

A: This is not a weight loss program, but participants who adopt new healthy lifestyle habits may lose a modest amount of weight. A modest weight loss of 5-10% of your current weight (e.g. 11-22 pounds if you weigh 220 pounds) is a realistic goal in this program.

Q: Who can join?

A: Anyone who is 18 years or older wanting to reduce cardiovascular risk that is eligible to use the Fond du Lac Health Service can join.

Q: How much does it cost?

A: Indian clients are not billed for co-pays or deductibles. Non-Indian clients with FDL Insurance may use the Fond du Lac Health Service but will be billed for co-pays, deductibles and services not covered by their insurance plan. The Cardiac Risk Reduction Classes are free of charge.

Q: How do I join?

A: Registration is now open. Call the medical desk at 879-1227 and join today. We recommend you schedule your initial appointment and your provider appointment when you register to avoid a long delay between the two visits.
