Title: Public Health Nurse, Full Time
Location: CAIR
Supervisor: Public Health Nursing Coordinator
Closing Date: Permanent
Rate of Pay: $32.00 to $37.65 per hour

 

Position Responsibilities:

  • Participate in the planning, development and implementation of agency programs.
  • Monitor equipment; supply use, and inventories related to Public Health Nurse Scope of Services.
  • Provide primary and secondary care to clients in the Indian community that will include: immunizations, monitoring of chronic diseases, screening clients, home visits, discharging planning, assessment and referral, and client and community education.
  • Network Fond du Lac Human Services Division services with other agencies and programs.
  • Write and monitor home-care contracts and supervise Health Care Assistants.
  • Identify and evaluate community resources. Under supervision adapts hospital procedures to home situation to give skilled care to patient regarding part-time or intermittent professional services.
  • Teaches and supervises family members or others giving care. Initiates nursing measures to prevent complications or to minimize the disabling effects of disease or injury.
  • Observes, evaluates, and reports to physical client’s physical and emotional conditions, reaction to drugs and treatments.
  • With increasing facility, interprets to primary care provider social and physical factors in the environment that affect client care. Interprets to the patient and family the implications of the diagnosis and the nature of the treatment consistent with the actions and wishes of the physician.
  • Performs diagnostic tests as authorized and interprets findings of tests to individuals and families.
  • Obtains laboratory specimens when indicated.
  • Gives preventive immunizations or treatment under medical supervision.
  • Shares in procedures designed to prevent and control the spread of communicable disease.
  • Applies measures for prevention and control as required by local health regulations.
  • Interprets local health regulations and emphasizes individual and group responsibility for reporting of any signs of communicable disease, secures prompt medical examination of individual who are suspected to having been exposed to communicable disease.
  • Plans for coordinated nursing service for individuals and families under their care.
  • Maintains necessary records for analysis and planning of service and for the establishment of priorities for care.
  • Uncovers health problems through observation, interviews, and analysis of records.
  • Teaches basic principles of healthful living in relation to changing needs of individual in all age groups.
  • Adapts and applies information from related fields, such as nutrition, safety education, psychiatry and dentistry.
  • Provides families with information, support and encouragement, which may help them to adopt attitudes and practices that, promote health and reduce anxiety, tension and fatigue.
  • Recognizes and interprets behavior patterns as influenced by basic needs for security, love, belonging to and receiving recognition from the group.
  • Contributes to the understanding and maintenance of wholesome family life, giving special attention to the founding and expanding family.
  • Consults about individual or family health problems with other professional workers such as social workers, nutrition consultants, psychologists, and psychiatrists.
  • Helps the family to accept appropriate medical, mile hospital, nursing home, welfare, or other care as necessary.
  • Interprets extent and limitations of services available.
  • Arrange referral and communicate pertinent information to the agency.
  • Has membership and participates actively on a Quality Improvement Committee.

Physical Requirements:

  • Normal physical requirements.

Position Qualifications:

  • B.S.N. is required.
  • Certified Public Health Nurse is required.
  • One year of public health nursing experience and one year of professional hospital nurse experience is required.
  • Experience in Indian community is desirable.
  • Willingness to accept philosophy, purpose and ideals of organization is required.
  • Ability to maintain confidentiality of staff/client information is required.
  • Valid Minnesota driver’s license and reliable vehicle is required.
  • Personal qualities and abilities to deal with clients and families are required.
  • Knowledge of and ability to apply nursing principles and practices is required.
  • Ability to assemble, maintain records, and present data in report form is required.
  • Ability to work with patients and families is required.
  • Ability to meet and deal with the public is required.
  • Ability to check and assign work to auxiliary workers is required.
  • Knowledge of agency procedures is required.
  • Ability to communicate effectively orally and in writing is required.
  • Ability to work independently and establish priorities is required.
  • Attention to detail and accuracy is required.
  • Subject to pre-employment annual background checks, and annual reference checks.
  • Subject to testing in accordance with the Fond du Lac Employee Drug and Alcohol Policy.
  • Subject to immunization review and submitted proof of required vaccinations.
  • Some travel may be required.
